Customer Credit Application and Contract

Please mail or fax a copy of your current sales tax permit with application.

Terms of Sale: Payment terms for all accounts require all charges incurred by the 25th of the month to be paid in full by the 10th of the following month. A late fee will be added to any balance past one month of age when statements are run on the 25th of the month. This fee will be calculated at 1.5% of the past due balance.

If failure to pay according to the terms of the agreement causes this account to be assigned or referred to an attorney for collection, the buyer agrees to pay seller’s reasonable collection and/or attorney fees and all court costs.

In order to obtain credit, I hereby guarantee the payment of all debts incurred by the above noted company even if the invoice is made out to a corporation or partnership.
